Brake discs/ pads recommendations

Hi

I have recently discovered some rear end vibration under heavy braking, so I am looking for recommendations for replacements brake discs and pads to fit inside standard 16 Inch Fuchs.

I am looking for any improved braking , and will probably upgrade the fronts at the same time.
